Care programs

Chronic care, remote monitoring, transitions, behavioral health and community support. We run the enrollment, outreach, queues and documentation behind each program — your clinicians stay in charge of care.

Each page explains who is eligible, what your patients experience, what our team runs, and the code set to confirm with your billing team.

Not sure which program fits?

Most practices run two or three programs together. Tell us who walks through your door and we'll point you to where to start.

How every program runs

We pilot with a defined patient group, prove the workflow, then scale — so nothing is promised before it works in your systems.

What patients notice

Patients feel the difference. Practices see it in the records.

For patients

  • A named contact between visits
  • Help with refills, rides and referrals
  • Devices set up for them
  • Fast follow-up after a hospital stay

For your practice

  • Consent and enrollment records
  • Time logs by staff type
  • Escalations routed under your protocols
  • Monthly summaries ready for sign-off

Clinical assessment, care-plan approval and treatment decisions remain with licensed clinicians. Reimbursement depends on payer rules and documentation; Carevory does not guarantee payment or clinical outcomes.

FAQ

Everything practices ask before a first pilot group goes live.

Several combinations are allowed — for example RPM alongside CCM, or BHI alongside CCM — as long as each program's requirements are met and the same time isn't counted twice. We map what applies to your population before launch.

Your clinicians, or qualified clinical staff working under your supervision and protocols where contracted. Carevory runs the operations around them: eligibility, enrollment, outreach, scheduling, queues, documentation and reporting.

No. Payer rules, documentation and clinical decisions determine payment. We help you meet program requirements and give you clean records for billing review.

It depends on system access, staffing and how many patients are approached first. We start with a defined pilot group and scale once the workflow is proven.
