Build the due list
We identify patients eligible for an initial or subsequent AWV.

The AWV is a yearly prevention-planning visit — risk assessment, screening schedule and goals. Most practices leave many eligible patients unscheduled. We find who is due, book them, collect the health-risk assessment ahead of time and prepare the chart so the visit runs on time.
Eligibility is confirmed against your own population before a pilot group is approached.
Medicare Part B beneficiaries past their first 12 months of coverage
No initial preventive exam or AWV within the past 12 months
A health risk assessment completed as part of the visit
A written personalized prevention plan provided to the patient
Your clinicians keep every clinical decision. We run the operations around them.

Calls, texts and letters invite patients through your approved channels.
Health-risk questions are gathered before the appointment where possible.
Open care gaps, screenings and immunizations are summarized for the clinician.
Ordered screenings and referrals are tracked after the visit.
